Clinical Rotations, Electives and Call Schedule

Required ICU and Elective ICU Rotations

As specified by the ACGME, there will be at least nine months of ICU rotations. Our fellow will do 7-8 months of SICU rotations (approximately to account for vacation time). One month will be a required Cardiac SICU rotation at the nearby Einstein campus (Jack D. Weiler Hospital) of the Montefiore Medical Center system. The Fellow can choose a one month elective in the Jacobi Burn ICU, MICU or extra SICU time.

Trauma Rotations

To help fulfill our education mission to be a trauma- and acute care surgery based critical care fellowship, we ask the Fellow to rotate on the trauma service for 2 months. The Fellow will collaboratively lead the Trauma service with the PGY4 Trauma Chief resident while on the rotation. As the year progresses and the Fellow becomes familiar with the Jacobi system, we will expect the Fellow to effectively act in a junior attending role to support the PGY-4—taking the PGY4 through operative cases, support the PGY4 during trauma resuscitations, etc.

Other Elective Opportunities

As basic echocardiography skills become increasingly important to the surgical intensivist, we want to ensure that our Fellow graduates with some competency in Basic Critical Care Echocardiography. Time will be arranged for the Fellow to spend time with cardiology faculty and the echocardiography technicians along with regular didactic and practicums from the SICU faculty who used echocardiography regularly.

With the emerging importance of REBOA, we want ensure the Fellow has a mastery of vascular access skills beyond the placement of central and arterial lines. The Fellow is invited to participate in any of the elective endovascular procedures done within our cardiac catherization laboratory.

The Fellow will also have the opportunity to work with anesthesia faculty to practice their airway skills and obtain the ACGME required number of intubations.

Call Schedule and Vacation

The Fellow will take an in house call approximately once a week with a schedule that provides for a “golden weekend” at least once a month. The majority of the calls will be for the SICU except for approximately once a month a call will be solely for the trauma service. During that “trauma call” the Fellow will take it alone or to support the PGY4 ideally as effectively a junior attending.

The Fellow will have 4 weeks vacation that they can take in 2 week or 1 week blocks as she/he chooses.
