Integrated Behavioral Health Care and Administrative Psychiatry

The Montefiore Care Management Organization (CMO), in a unique partnership with Psychiatry and Bronx Psychiatric Center offers an innovative psychiatry fellowship in Integrated Behavioral Health Care and Administrative Psychiatry. Montefiore is a national leader in population health and the integration of behavioral healthcare and primary care and care management services. Montefiore was one of the 32 Pioneer Accountable Care Organizations, and is the recipient of a $5.6 million CMMI Innovation Grant to implement integrated care in a safety net population. The fellowship is a one-year commitment with an option to renew for a second year. The fellow will spend three days at integrated primary care and behavioral health sites seeing complex patients for diagnostic evaluations and pharmacology. The fellow leads the site’s weekly team meetings and completes chart reviews to advise primary care providers and specialists using a measurement-based, treat-to-target model with all treatment elements captured in a registry embedded in the electronic medical record. In addition to depression we treat anxiety, PTSD and substance use problems. Our patients include newborns and their parents, children, teens, adults, and older people. The fellow’s other two days include: 1) Data analysis, review and evaluation of outcomes from the Bronx Behavioral Health Integration Project, 2) An individual project, including materials development, trainings, and outcomes measurement, 3) Behavioral telehealth interventions, 3) Use of population behavioral health principles for quality improvement.
