The Montefiore Medical Center Department of Psychiatry Out-Patient Department (OPD) is the major ambulatory care training site for both the Adult and Geriatric Residency Programs.
The educational objectives at this site are achieved through the resident’s assignment of cases in the psychiatry outpatient clinic and consultations to patients cared for by the geriatric medicine faculty and staff.
Dr. Scalmati who supervises the geriatric component of the psychiatry outpatient department provides weekly supervision to each of the fellows, during which the fellow’s proficiency with Core Competencies can be assessed.
Fellows present any new intake to the weekly multidisciplinary treatment team. Fellows also present on a rotational basis during a weekly seminar where they will be expected to present a literature review and discuss evidence based practice on selected topics pertinent to their clinical work.
Fellows also supervise second year residents for a monthly intake at the outpatient clinic.
Margaret Tietz Center for Rehabilitation and Nursing Care (MTC) is a teaching nursing home located one half hour from the Medical Center in Jamaica Queens.
The educational objectives are achieved through the assignment of nursing home residents to the trainee for evaluation and ongoing treatment, through twice monthly conjoint formal presentations with trainees in geriatric medicine to Dr. Kennedy and through individual supervision.
These presentations are organized by a full time coordinator of psychosocial services and attended by the facility’s medical director, director of social services, activities therapist, chief dietitian, nursing educators, staff social workers and nurses. It is the preparation, presentation and debriefing associated with this presentation through which the resident’s proficiency with Core Competencies can be assessed.
Montefiore Home Health Care Agency and Co-op City Naturally Occurring Retirement Community (NORC) are the sites of the community based experience.
The Home Care agency is one of the largest service providers in the Bronx and one of the few in the United States with an integrated geriatric mental health component.
The Naturally Occurring Retirement Community at Co-op City houses 8000 older adults in high rise apartments overlooking Eastchester Bay.
The educational objectives are achieved through assignment of home care visits to patients cared for by the geriatric medicine faculty and staff (specifically as consultant to the Geriatric Medicine Home Visiting Program and attendance at their weekly rounds along with the geriatric medicine fellows). Dr. Colemon, as consultant to the Montefiore Home Health Agency and consultant to the NORC at Co-op City, provides supervision.
Fellows have the advantage of being able to follow patients over time while they are cared for by the visiting nurses as well as interfacing with geriatric medicine colleagues and community based agencies.
