The Montefiore Asthma Center was created in 2011 to bring a multidisciplinary team approach to bear on refractory asthma for patients who have frequent emergency department (ED) visits or admissions due to asthma exacerbations. The Asthma Center's team includes allergists, pulmonologists, respiratory technicians and asthma health educators who evaluate the patient carefully to confirm the diagnosis of asthma and devise a tailored treatment approach. The team then communicates its findings and recommendations with the primary care physician who will continue the patient's longitudinal asthma care. This program is a joint venture between the Department of Medicine and Montefiore's Care Management Organization.  

Outcomes from the first year suggest that the Asthma Center can help reduce respiratory-specific hospital admissions and ED visits.
