Restricted hospital service on weekends, when medical staff are often unavailable to implement discharge decisions, has historically prolonged hospital stays unnecessarily for many patients. Montefiore Medical Center responded to this issue with a dedicated weekend signout initiative.
Every Friday, house staff and physician assistants (PAs) review and identify patients who may be medically ready for discharge over the weekend. This effort provides continuity for weekend house staff to appropriately discharge patients on Saturdays and Sundays, thereby reducing the number of unnecessary hospital bed days.
This initiative has significantly increased the number of patients discharged from the hospital each weekend, improved effectiveness of care, and reduced bed days and costs.
