With the increasing use of advanced technology systems in hospitals, healthcare professionals must be aware of the challenges of technology and be vigilant in computerized ordering and prescribing. Identifying the correct patient prior to placing an order in the computer system is imperative to prevent wrong-patient medication errors.
The patient safety team at Montefiore Medical Center understands the risk of placing orders under the wrong patient profile and is working to implement a fail-safe step for providers to verify patient identity before any order set is processed in the computerized order entry system.
