Quality Improvement in Diabetes Care

Inpatient Initiatives

Glycemic Management in the Acute Care Setting
The Montefiore Clinical Diabetes Program focuses on quality improvement through insulin therapy for glycemic control, consistent carbohydrate meal options, bedside glucose monitoring and A1c tests, targeted insulin educational materials, and e-learning modules for physicians and nurses.

Post-AMI Hyperglycemia Control
Einstein-Montefiore Cardiology and Endocrinology investigators are currently conducting to determine the effects of close glycemic monitoring and control on outcomes of patients with acute myocardial infarction (AMI).

Outpatient Initiatives

Sweethearts Quality Improvement Program
The Sweethearts QI Program, a comprehensive, multidisciplinary diabetes outpatient treatment program, incorporates enhanced delivery system design, decision support, self-management support, and clinical information systems to effectively treat patients with chronically uncontrolled diabetes.

Research Initiatives

Collaborative Clinical Diabetes Studies
Einstein and Montefiore collaborate seamlessly to develop community-centered diabetes management programs and research studies.
