Objectives - After attending this activity, participants will be able to:

  1. Define patient centered medical homes (PCMHs)
  2. Review the published literature of PCMHs
  3. Describe the evoluition of our resident teaching practices in becoming PCMHs
  4. Describe specific initiatives underway at our resident teaching practices as part of their changes to transform into PCMHs
  5. Review the new residency program rotation and curricula of the PCMH
