Preparation of application
A complete application includes:
- Completed application form
- Personal Statement
- Three letters of recommendation
- A photo
Applicants who have recently completed training are expected to request letter of recommendation from the program director of their training program.
Applicants with clinical practice experience should provide letters of reference from clinicians in their practice community who are familiar with their skills.
