The variety of data sources
integrated and the robust datasets with the multitudes of variables that are
extracted and acquired for the over 2,500 HIV/AIDS patients followed at
Montefiore, present a challenge for information retrieval, modeling and processing
of such multi-source data. The web-based CFAR Clinical Research Informatics
System (CCRIS) utilizes Reporting Services to provide a sophisticated and
user-friendly interface for the presentation of data extraction results in
order to facilitate quick and easy access, retrieval, filtering, reporting,
sharing, and processing of information to CFAR investigators engaged in
HIV/AIDS research. Summary data are presented by category (demographic,
medication, medical history, laboratory results, cancer diagnoses etc.) and
presented as basic statistical summary data tables (cross tabulations, means
etc.) which allow investigators to drill down through categories of patient
data. The reporting feature is interactive and allows investigators to expand
all statistical tables into new categories or collapse them in order to extract
more specific information. All selected data can be viewed or extracted to a
number of formats for statistical analysis.
An additional
feature of the CCRIS is the
Cohort Builder. This feature allows a
researcher to select a cohort from the total group of HIV/AIDS-infected and
at-risk patients followed at Montefiore based on selection of multiple
criteria. By clicking on the Cohort Builder button in the lower left of
the main CCRIS page, the user is brought to the
Cohort Builder itself. At the far right
of the Cohort Builder screen is the Parameters Section where a series of
Criteria Categories are displayed. From each Criteria Category a researcher can
select one, multiple or all choices for that Criteria Category. For example,
from the AIDS Statuses category, the researcher may choose to see patients who
are 1) known AIDS positive, 2) known AIDS negative or 3) of an unknown AIDS
status or any combination of these choices.
After selecting all desired criteria from the available categories, the
researcher clicks the ‘Apply’ button in the lower right corner of the
screen. The system then calculates the cohort that matches the selected
criteria and displays the cohort count on the left side of the screen.
This cohort count is further broken down by the characteristic chosen as the
‘Breakdown by:’ variable specified at the top of the Parameters Section. The
center of the screen summarizes the criteria on which the selection of patients
were based. By selecting the ‘View
Demographics’ or the ‘View Labs’
link under the cohort counts, the appropriately credentialed researcher is able
to obtain demographic and lab result information for the selected cohort.
This detailed information can then be exported to Excel for further analysis.

Selecting
‘MEDICATIONS’ from the main menu will display a list of categories of
medications by year prescribed and year discontinued. This table can be
further expanded by clicking on a category of medications (e.g. ARV) to view
patient counts for specific ARV medications.
Selecting
‘LABS vs. TREATMENT’ from the main menu will display a list of most recent
Viral Load values for the population, categorized by range. Clicking on a
VL range will expand to display associated CD4 ranges for those
patients. Clicking on ART treatment will expand the table to display
patient counts based on HIV status. This table can be further
expanded by clicking on any cell to display the cohort in that cell with more
explicit clinical information. This cohort can be now be exported to excel or a
variety of formats for import into a statistical package for analysis.
