APPLICATION:
CAYUSE FORMS (All Einstein applications are, at minimum, internally routed through the Cayuse system. Federal applications are submitted by downloading the required forms associated with a particular Funding Opportunity Announcement (FOA) into Cayuse (if they are not already there). Please make sure that you choose the correct FOA to respond to, as it makes a difference in the submission of the application to the NIH).
NIH FORMS/INSTRUCTION MANUALS (Most NIH applications are submitted electronically. A few mechanisms -- at this time the "P" series of grants -- still require paper submissions. This link provides generic paper forms, as well as instruction manuals to be used in submitting any NIH application. SF424 Application Guide should be used when assembling an NIH application).
INTERNAL ROUTING FORMS:
The following 3 forms are mandatory for each NEW, RESUBMISSION, RENEWAL and NON-COMPETING proposal:
PI-PD Certification (required attachment for every proposal)
Proposal Summary Part 2 (required attachment for every proposal)
EH&S "Short" Form (required attachment for every proposal)
Download documents to your hard drive--complete, save and attach to the Document section of Cayuse.
NO proposal will be allowed to go through routing without these documents filled out COMPLETELY.
COMPLIANCE DEPARTMENTS' FORMS:

For those investigators who do NOT have current EH&S forms on file. In that case, fill out the EH&S short form (above) and the DOR/Health and Safety Evaluation form found on the EH&S website.
Animals:
IACUC Grant Application Page (required of all grant applications involving vertebrate animals)
IACUC for Non-Competings, renewals, revisions and supplementals
IACUC Memo(regarding required IACUC forms for different submission types)
Human Subjects:
