Like the Invention Disclosure Form, the Research Tool Disclosure Form is used to capture information on research tools developed at Einstein.
The Research Tools Form serves several important purposes including providing the information needed to: establish a legal record of your research tool discovery; assist in assessing its potential market value; and reporting any commercially applicable discoveries to granting sponsor (i.e., NIH).
Please note, this form is interactive and questions will change depending on what type of tool you are disclosing. Below are a few instructions regarding submitting information on your research tools to this Office:
- Please submit one form for each research tool.
- Once the form is completed, please click the “Print and Sign” button. After the form is printed, please sign and date it.
- Submit the printed copy, which you signed and dated, it to the Office of Biotechnology (Belfer Building Room 908 or fax it to us at: 718-430- 8938).
If you would like to update our staff that the form is coming, you can also use the “Send Copy” button and a copy of the form will be emailed to the office. But we will still need a signature copy from you, so please be sure to use the “Print and Sign” button first.
